The Heart Hospital of Lafayette offers a competitive benefits program that is a part of our employees' total compensation program. We believe that our benefits program is valuable to our employees because it offers protection from catastrophic health-related financial issues. Because many of the benefits are paid for with before-tax dollars, the employee also realizes a tax savings.
Health Benefits
- A PPO plan that allows you to see a specialist without a referral from a primary doctor.
- A vision plan that provides a dollar allowance for routine eye exams, glasses, and contacts.
- A prescription drug plan, which includes a mail-order Rx component.
- A dental plan. You select whether the Preventive Care coverage level or the Comprehensive Care coverage level is the best fit for you.
- Health Care and/or a Dependent Care Flexible Spending Accounts. You may enroll in one or the other . . . or both!
Disability Benefits
- Voluntary Short Term Disability plan. Because you pay the premium, any benefit that you receive is tax-free!
- Voluntary Long Term Disability plan.
- Full-time Employees: The hospital pays 50% of the cost of this benefit.
- Part-time Employees: Because you pay the premium, any benefit that you receive is tax-free!
- Voluntary Long Term Care plan. An individual policy is available to you, your spouse, your parents, and your parents-in-law at low group rates.
Life Insurance Benefits
- Company paid Group Term Life and AD&D Insurance. This will provide a benefit to your survivor in the event of your death.
- Voluntary Life Insurance is also available for you, your spouse, and your children.
- Voluntary Accident Insurance. This benefit is separate from the voluntary life benefit, and you may elect the voluntary accident insurance even if you do not have life insurance. This benefit pays out in the event of your (or a family member's) death by accident.
Retirement Benefit
- We offer a 401(k) Profit Sharing Plan. This plan allows you to save up to 25% of your income on a pre-tax basis. The Heart Hospital of Lafayette matches 25% on every dollar you contribute, up to 6%.
- You are eligible to participate in the plan immediately.
- The company match is also effective immediately.
- You are always 100% vested in the company match portion!
Work / Life Benefits
- We have a Paid Time Off benefit that provides days to use for vacation time, holidays, and sick time.
- A Section 529 College Savings Plan is available to you. With this plan, you can save for your child(ren)'s college education through the convenience of payroll deduction.
- A Tuition Reimbursement Plan will help offset the cost of your secondary education.
- We offer you the optional benefit of enrolling in Hyatt Legal Plan. Participation in this plan offers you access to a network of attorneys who provide many services at no additional cost. You can use the Plan for closing on a home, drafting a will or living trust, and many other legal issues.

- Homeowners, renters, vehicle, and boat insurance is available from a national company. You pay your premiums via payroll deduction.
